with Kōena Integrative Medicine ("Kōena," "we," "us,"

or "our") electronically, you agree to be bound by these

Terms, please do not use our Site or services.

These Terms apply to all visitors, patients, and users

of our Site.

2. NOT A SUBSTITUTE FOR MEDICAL CARE

THE INFORMATION PROVIDED ON THIS WEBSITE IS FOR

INFORMATIONAL PURPOSES ONLY AND DOES NOT CONSTITUTE

MEDICAL ADVICE, DIAGNOSIS, OR TREATMENT.

Nothing on this Site should be used as a substitute

3. NO PROVIDER-PATIENT RELATIONSHIP

Submitting a form, inquiry, or contact request through

this website does not establish a provider-patient

relationship with Kōena Integrative Medicine or any

of its providers. A provider-patient relationship is

only established through an in-person or telehealth

consultation and mutual agreement to proceed with care.

4. SERVICES

Kōena Integrative Medicine offers integrative medicine

services including but not limited to functional medicine,

acupuncture and Eastern medicine practices, hormone

replacement therapy, GLP-1 weight management programs,

massage therapy, aesthetics and skin care, injection

therapy, light therapy, and mind-body practices.

All services are subject to clinical eligibility

requirements and provider discretion. Submitting an

intake form does not guarantee candidacy for any

program or service.

5. ACCURACY OF INFORMATION

You agree to provide accurate, current, and complete

information when submitting any form or communication

through our Site. You acknowledge that providing

inaccurate or incomplete medical information may

affect the quality and safety of your care and that

Kōena Integrative Medicine is not liable for adverse

outcomes resulting from inaccurate or withheld

information.

6. ONLINE FORMS & ELECTRONIC CONSENT

By submitting any intake or consent form through our

website, you acknowledge that:

- Your typed name constitutes a legally binding

electronic signature under the Electronic Signatures

in Global and National Commerce Act (E-SIGN Act)

- You have read and understood all information

presented in the form

- You are voluntarily providing consent where requested

- You are at least 18 years of age or have parental/

8. PRICING & PAYMENTS

All pricing listed on our website is subject to change

without notice. Prepaid program pricing is an estimate.

Additional costs may apply based on individual clinical

need, medication requirements, or additional services.

Prepaid programs are non-refundable once medication

has been dispensed or services have been rendered.

All sales of prepaid programs are final.

Kōena Integrative Medicine does not accept insurance

for most services. All fees are self-pay unless

otherwise stated.

9. CANCELLATION POLICY

We require a minimum of 24 hours notice to cancel

or reschedule an appointment. Late cancellations

or no-shows may be subject to a cancellation fee.

All content on this Site including text, graphics,

logos, images, and other materials is the property

of Kōena Integrative Medicine and is protected by

applicable copyright and intellectual property laws.

MOBILE TERMS OF SERVICE

1. PROGRAM DESCRIPTION

Kōena Integrative Medicine offers SMS/text message

communications to patients and subscribers who have

opted in to receive mobile messages. Messages may

include appointment reminders, health and wellness

information, promotional offers, clinic specials,

program updates, and other communications related

to your care and our services.

2. HOW TO OPT IN

You may opt in to receive text messages from Kōena

Integrative Medicine by:

- Providing your phone number and checking the SMS

consent box on any of our website forms

- Texting a designated keyword to our SMS number

- Verbally consenting and providing your phone number

to a Kōena team member

By opting in, you confirm that you are the account

holder or have the account holder's permission to

enroll the phone number provided.

3. HOW TO OPT OUT

You may opt out of receiving text messages from

Kōena Integrative Medicine at any time by:

and will not exceed applicable carrier guidelines.

6. MESSAGE & DATA RATES

Message and data rates may apply. Kōena Integrative

Medicine is not responsible for any charges incurred

from your mobile carrier related to SMS communications.

Please contact your mobile carrier if you have

questions about messaging rates.

7. SUPPORTED CARRIERS

Our SMS program is supported by most major US carriers

including but not limited to AT&T, Verizon, T-Mobile,

Sprint, Boost Mobile, and US Cellular. Carrier support

is not guaranteed for all carriers.

8. PRIVACY OF YOUR MOBILE NUMBER

Your mobile phone number and SMS consent data will

never be sold, rented, or shared with third parties

for their own marketing purposes. Your number is

used solely to communicate with you on behalf of

Kōena Integrative Medicine.

All mobile communications are handled in accordance

with our Privacy Policy, available at:

9. PROTECTED HEALTH INFORMATION (PHI)

Standard SMS messaging is not a HIPAA-secure method

of communication. Please do not send Protected Health

Information (PHI) including medical records, lab

results, diagnoses, or sensitive health details

via text message.

For secure communication of PHI, please use your

Cerbo patient portal. If you have not yet been set

up with your patient portal, contact us at

10. COMPLIANCE

Our SMS program complies with all applicable laws

and regulations including the Telephone Consumer

Protection Act (TCPA), the CAN-SPAM Act, CTIA

guidelines, and carrier requirements.

11. CHANGES TO THESE TERMS

We reserve the right to update these Mobile Terms

of Service at any time. Changes will be posted on

this page with an updated "Last Updated" date.

Continued use of our SMS program following any

changes constitutes your acceptance of the

updated terms.
